Company Logo Missing from Invoices

3.0.11, again ...

I uploaded a logo file way back in 2.x and it was working fine. Now, I can still see the logo filename when I go to Settings > Company but I can't save or cancel that page because the Save/Cancel buttons are missing (as noted in a separate thread). Further, when I make an invoice, my company's logo is missing from the PDF.

Update: Issue was caused by a failed upgrade from 2.x to 3.0. I had to completely start over with a new install of 3.0.11 and waste over 20 hours rebuilding all my data and settings.
