install fails after step 3, create user

My install seems to go fine, but after I attempt to create the master user account the next screen gives me this.  

"You don't have permission to access /install/settings on this server."

 

Then I try to log in its says bad user and resetting my password gives me a no user found error. 

Looking in the DB I see some tables have been populated but nothing in the _accounts DB which I am guessing where user might be?  

This seems to be falling down when new users are added.  

 

Suggestions?

That's a general server error, ask your hosting company for the details.
